How to sign up for a MyChart account:

Request an activation code. This can be done online, in the mobile app, or by calling
Customer Service.

On the MyChart website, visit mychart.myhfhp.org:
• Select “Sign up.”
• Enter the activation code if you received one or select "Sign up with your information." 
• Enter your required personal information. 
• Select “next” and follow the prompts.

On the MyChart mobile app:
• Select “Add organization.”
• Select “Health First Health Plans.” 
• Tap the results and select "Add selected organizations."
• Select "Sign up."
• Enter the required personal information, select “next” and follow the prompts.
